In a 55-year-old woman, on the arm

 

What to see at dermoscopy:

-multicomponent pattern
-irregular and broadned pigment network
-globules
-blotches
-hypopigmented and structureless area
-blue-white veil
-regression areas (scar-like depigmentation)
-erythema
-red lacunae on the left side of lesion (second dermascopic picture)
Excision was indicated and histology confirmed the diagnosis of melanoma in situ

Comments: the image of red lacunae might correspond to a cherry hemangioma (collision tumor).

 

Patient 163

In a 71-year-old woman, on the index finguer

What to see at dermoscopy:

-irregular diffuse pigmentation
-dots
-at some areas, the parallel furrow pattern is respected

Biopsy was indicated
Dermoscopy in favor of acral  melanoma

Comments: the presence of irregular diffuse pigmentation on palms or soles raise the possibility of invasive melanoma.

In a 53-year-old woman, multiple lesions on the sole. 

What to see at dermoscopy:

First picture corresponds to double hyperkeratotic papules with a central black dot (under the nail plate). At the second picture, the right lesion was gently shaved and white ovoid structures over the skin were observed which corresponds to eggs.
Dermoscopy in favor of tungiasis


Comments: There is upcoming evidence that dermoscopy facilitates the in vivo diagnosis of skin infections and infestations (viral warts, molluscum contagiosum, scabies,

In a 53-year-old man, on the back

What to see at dermoscopy:

-multicomponent pattern
-pseudopods
-blue-white veil
-melanosis or peppering
-negative pigment network
-chrysalis structures
-erythema

Excision was indicated
Dermoscopy in favor of
melanoma

Comments: chrysalis structures are well represented in the second picture.
Those white shiny streaks were first discribed in 2009 and their presence in melanoma correspond to changes in the composition and orientation of collagen in the underlying stroma.
